A 56 year old
gentleman who presented with abdominal pain had been experiencing
diarrhoea and vomiting for the last 8 days. He had been unable to keep
any fluids down over the last week.
He was a little
confused/disorientated at times, he was also complaining of limb
stiffness and cramps.
His ECG is below,
what are your findings?

Case 8
A 83 year
old lady presented to A&E with a history of chest pain. It had started
while she was in bed at 6am. On arrival she scored it at 3/10, she had
felt short of breath. By late morning she had arranged to go and see her
GP who sent her to A&E.
She
attended A&E at 13:30.
What are
your ECG findings?
Would you consider
thrombolysis?
2 hrs later she
dropped her BP, her ECG now looked like this.
On further
examination she had developed a pan systolic murmur.

A 76 year old Asian
lady presented in A&E at 07.40hrs with a 48hour history of chest pain,
severe difficulty in breathing and coughing up pink frothy phlegm, which
was suggestive of left ventricular Failure (LVF)
PMH Diabetes
Hypertension
On arrival she was
pale and sweaty, became bradycardic and went in to cardiac arrest.
She was given CPR
and adrenaline, after 2 ˝ minutes she recovered her output, and was
breathing for herself.

presenting ECG prior to her arrest, what is your interpretation?

A 62year old
gentleman presented following chest pain the day before, which was
“pumping “ in nature. It started in his neck and radiated to the centre
of his chest and down both arms. The pain was currently 5/10. The pain
had started at 15:00hrs yesterday while gardening and was associated
with SOB, sweating and nausea.
