Case 30

A 40 year old man had experienced chest pain radiating down both arms since1pm.He described it as a heavy tightness in his chest, while at rest.

Pain rated as 8/10. He felt sweaty with mild shortness of breath.

PMH: ? Raised BP 

What are your ECG findings?

Case 31

A 55 year old male presented with a dull central chest ache since 9.30am. The pain radiated to his left arm. He was sweating, vomiting and looked pale. 

O2, aspirin and GTN had been given by the paramedic. 

What are your ECG findings?

What is your treatment?

Would you thrombolyse?  

 

Case 32

A 53 year old lady with known COPD, Type 2 diabetes and hypertension developed chest pain lasting for one hour. It was retro-sternal and radiated to both arms, ECG shows intermittent LBBB

He had a stroke 10 years ago and had been admitted 14 days ago with a positive  Troponin of 0.14

Had a positive ETT a week ago after just 7 mins on the treadmill and was waiting for an inpatient angiogram. 

What are your ECG findings?

What is your treatment plan?  

 

Case 33

A 42 year old male presented at 6.30am in A&E with chest pain which radiated to his left shoulder and in to his arm. 

The pain was associated with difficulty in breathing and sweating. He had no nausea or vomiting.

He described the pain as sharp and tight and rated it at 7-8/10

It was worse on inspiration.

The pain had been on and off for 1 week, but had started at 5am while at rest and was worse than previous episodes.

He had a weeks history of shortness of breath on exertion.  

He was known to be an IDDM (insulin dependant Diabetic) 

What are your ECG findings and treatment?  

 

Case 34

A 70 year old asian lady presented with a 24 hr history of pain between her shoulders. The pain was epigastric in nature and radiated down her left arm. She complained of SOB, nausea. No sweating

GTN taken, which eased symptoms but then returned.

PMH MI x 2 CABG 16 yrs ago

IDDM

Permanent pacemaker in 2000

Case 35

A 35 year old male presented with sudden onset of central chest pain, right sided chest pain, no radiation. He was sweaty, no complaints of nausea. 

He was has been an IDDM for 23 years, no other risk factors. 

On admission he was pale, cold and clammy. 

This was ECG on arrival, what are your ECG findings?

What would be your treatment plan?
